The latest figures from Public Health Scotland reveal continued progress in reducing NHS waiting lists, with patients waiting more than 52 weeks decreasing for the eighth consecutive month.
As of 31 January 2026, approximately 586,859 individuals were on at least one new outpatient, inpatient, or day case waiting list. This equates to roughly one in nine of Scotland’s population. I have covered health policy throughout my career, and while these numbers remain significant, the downward trajectory represents genuine progress.
Long Wait Reductions
The most encouraging aspect of the data concerns the longest waits. New outpatient waits exceeding 52 weeks have more than halved since July 2025, decreasing by 52.6% over that period. At the end of January 2026, the total number of patients waiting more than a year stood at 23,057, down 1,814 from December.
Perhaps most significantly, the number of patients waiting more than two years has fallen to 3,639, a reduction of 620 from the previous month. These extended waits cause the most distress to patients and their families, so their reduction represents meaningful improvement in people’s lives.
Government Targets
The Scottish Government has committed to ensuring no patient waits longer than 52 weeks by March 2026. With that deadline now imminent, NHS boards across Scotland have been working intensively to meet the target. However, some health policy analysts question whether this ambitious goal will be fully achieved given the current trajectory.
Recent months have seen substantial investment in additional capacity, including weekend clinics and the use of private sector facilities to clear backlogs. Health boards report that staff have responded positively to these initiatives, despite the additional workload involved.
Performance Variations
Performance varies significantly across different health boards and specialities. Some boards have virtually eliminated waits over 52 weeks, while others continue to face substantial challenges. Orthopaedics and ophthalmology remain particular pressure points, with demand consistently exceeding capacity.
Looking ahead, NHS Scotland faces the dual challenge of maintaining progress on waiting list reduction while preparing for typical seasonal pressures. The test will be whether recent improvements prove sustainable when the health service faces its next major demand spike.
